Social Workers

Making the decision to move into a long-term care facility can be difficult for both the resident and their family. Whether coming to us for short-term rehabilitation or long-term care, we are here to provide support through the adjustment period and beyond, acting as a liaison between the resident, family, and facility. Our primary goal is to assist our residents in achieving optimal psycho-social well being and quality of life throughout their stay.

The facility has two trained professionals: Jackie Beaudoin, LBSW and Samantha Bannenberg, LLMSW. They wear many hats within the facility including monitoring of psychotropic medications, behavior management, discharge planning, cognitive, mood, and trauma screens, voting assistance, assistance with advance directives and guardianship's, and providing comfort and support through the end-of-life process. Our social service department works closely with multiple different community agencies to guarantee the highest quality of life for our residents whether they make the facility their permanent home or return to independent living within the community.
